A new randomized trial from Washington University, published in Cell Metabolism (August 27, 2026), compared three diets in people with obesity, prediabetes, and fatty liver:
• Ketogenic (4% carb / 73% fat / 23% protein)
• Mediterranean (50% carb / 35% fat / 15% protein)
• Very-low-fat plant-forward (70% carb / 15% fat / 15% protein)
All food was provided. Weight loss was matched at about 10%. Muscle insulin sensitivity improved about 50% on every diet. The liver and blood sugar did not treat the diets as equal.
Keto cut liver fat 67% vs 45% on the other two diets, improved liver insulin sensitivity two to three times more, dropped 24-hour glucose and insulin harder, and sent prediabetes into remission in about 50% of that group vs 29% Mediterranean and 7% low-fat.
