Among Popular Diets, Keto Is Best for Liver Health, Study Suggests

Howdy, tribe. Pete here with some fresh news off the grill.

A new trial published in Cell Metabolism put 55 adults with metabolically unhealthy obesity on one of three diets for about 5 months: low-carb keto, Mediterranean, or a very-low-fat plant-forward diet. Food was provided, and adherence was reported at 95% in every group.

Weight loss was about the same across the board (around 10%). But keto pulled way ahead on the liver:

  • Liver insulin sensitivity improved 2 to 3 times more on keto than on the other two diets

  • Liver fat dropped 67% on keto, vs. 45% for both Mediterranean and plant-forward

  • 24-hour glucose fell 20% on keto, vs. 8% for the others

  • Half the keto group reversed their prediabetes, vs. 29% on Mediterranean and 7% on plant-forward

Same weight lost, very different results under the hood. Makes you wonder what "a calorie is a calorie" folks make of that one.

So I've got to ask: has anyone here had a fatty liver diagnosis, or seen their liver enzymes (ALT/AST) or A1c change after going keto or carnivore? What did your numbers do?
